SMART® Health Card Verifier App
Verify an individual’s vaccination credential by using the free Verifier App and scanning a SMART® Health Card QR Code.
How it works
The SMART® Health Card Verifier app uses the phone camera to scan a SMART Health Card QR code to quickly verify an individual’s vaccination credential:
Verifies the SMART Health Card is valid and not tampered with
Confirms the SMART Health Card was issued by a trusted issuer in The CommonTrust Network Registry
Displays key information on the SMART Health Card: issuer name, vaccine type, dates of vaccine doses, recipient name and date of birth
One of the three results will appear:
Verified
The SMART Health Card scanned comes from a verified issuer in The CommonTrust Network’s registry of trusted issuers and was not tampered with.
Not verified
The SMART Health Card scanned is not valid, as it may have been corrupted, or modified after it was issued.
Issuer not recognized
The SMART Health Card scanned may be valid, but the issuer is not in the CommonTrust Network’s registry of trusted issuers. The issuer URL will be provided.
Please note: The SMART Verifier app only scans SMART Health Cards. It does not scan paper CDC cards or other types of health credentials. The SMART Health Card Verifier App does not store SMART Health Cards.
